Insurance

   Insurance can be complicated and confusing; however...it doesn’t have to be. Understanding the policies and benefits of your plan will greatly help you with your health care needs. As a courtesy to our patients, we will check your plan when you come in for your exam. Patients are welcome (and encouraged) to contact their insurance company for coverage and benefits prior to their visit as well.

   In most cases we are considered "in-network" with the following insurance companies/networks, as well as third-party entities that offer vision benefits. Click on the provider logo below to see your benefits:

Third Party Vision Plans:

   If you have a vision plan from an "out-of-network" company, we still welcome your visit. Although you will be required to pay in full at the time of your visit, you will be able to receive the benefit from your plan by filling out the appropriate form and mailing it to your provider. Once the form has been received and processed, they will send a reimbursement to you. Such plans include:

   Our office can bill most insurance companies as a courtesy to our patients, but cannot guarantee payment. Any un-paid fees or outstanding balances are the responsibility of the patient.
